File & Serve documents its own attendances and can provide service records based on what actually occurred. It does not select legal forms or give legal advice.
Factual record
Service Confirmation / Proof of Service
When an assignment or the included attempts are completed, File & Serve provides the applicable confirmation or proof record from its service history. The record should describe what File & Serve actually did and observed—not what a party hoped would happen.
Commissioned document
Affidavit when File & Serve completed service
A commissioned affidavit may be requested when File & Serve completed the service and can prepare the document from its own factual service record. File & Serve does not swear to an attendance performed by someone else.
Client responsibility
The required proof depends on the proceeding
The client must determine whether an affidavit, certificate, confirmation, or another proof document is required, which form is correct, who must sign it, and where and when it must be filed. File & Serve does not select official forms or provide legal advice.
Dates, times, address, recipient identification, manner of service, attempts, and exhibits must match the factual record. A requested wording change cannot alter what actually occurred.

Is the affidavit automatically included?
Review the current request options and pricing. A commissioned affidavit may be available as an add-on when File & Serve completed service and can prepare it from its factual record.
Can File & Serve choose my court form?
No. File & Serve does not select, complete, revise, or advise on official court forms for clients.
Can an unsuccessful attempt be turned into successful service?
No. The record must accurately state what occurred.
Who decides whether the proof is legally sufficient?
The court or tribunal applies its rules. Clients should obtain legal advice when they are unsure what proof is required.
